Here are the steps to follow:

• Pick-up a matching gift form at your local human resources department.
• Fill out and sign the employee section of the matching gift form.

Heart’s Home USA will pro­cess your matching gift form and send it to your employer. Your employer sends its matching gift dona­tion directly to Heart’s Home USA.
